For independent pharmacy owners

The trade news that moves your bottom line. Nothing else.

One email a week, written for the people who sign the payroll: PBM contracts and what they really cost, reimbursement math in dollars per script, legislation that changes your pay, audit and recoupment warnings, and one peer-tested tactic per issue. No vendor fluff. No chain-pharmacy narratives.

What each issue covers

The whole business of running an independent pharmacy.

Eight sections, every week, in the order an owner cares about them.

1

The lead

The single biggest money story of the week: PBM reform, contract changes, legislation. With the math, in dollars per script.

2

The contract table

What changed in PBM and payer contracts, what the fine print actually says, and what to do before you sign.

3

State watch

Reimbursement floors, anti-steering laws, ownership-separation bills, enforcement actions. State by state.

4

Closures and openings

Honest numbers on pharmacy closures and what they mean for the patients and markets left behind. No euphemisms.

5

Cash flow check

Payment delays, program rollouts, wholesaler terms. The stuff that decides whether the shelves stay stocked.

6

What owners are doing

One peer-tested tactic per issue: a niche revenue play, an appeals process that worked, a staffing move that stuck.

7

Audits and recoupments

New audit triggers, expanded audit scope, how PBMs are hunting recoupments now that DIR moved to point of sale.

8

The takeaway list

Three to five bullets. Act on these now. Everything else can wait.

Vendor-neutral by charter.

The trade association sells email blasts. Vendors write advertorials. The Indie Script sells neither: sponsors buy clearly labeled ad units, and they never buy the copy. Every factual claim is cited to a real source, and conflicts are disclosed in the issue.

If a vendor wrote it, you would smell it in the first paragraph. You will never have to wonder here.

From issue #1

A taste of the first issue.

Issue #1 is in draft now. Here is what is in it.

The lead

CMS is writing the rules that decide your pay. The PBMs are writing the loopholes.

Congress passed PBM reform. Now CMS is writing the implementing rules, and NCPA's July letter lays out what independents need: PBMs defined by behavior, paid a flat admin fee, and pharmacies reimbursed on true acquisition cost. The PBMs, per NCPA's CEO, "will try to have loopholes inserted into the rules." This is the fight that moves your margins.

The contract table

Cost-plus is here, and it is not what you were promised.

The FTC forced PBMs to offer cost-plus options. But the emerging contracts demand your confidential wholesaler invoices, strip out your off-invoice discounts by paying dead-net, and carry no mandatory dispensing-fee floor. One Orlando-area pharmacy took a 45% cut on a single script and lost $15.45 on the fill. Read before you sign.

What owners are doing

The niche revenue play.

The owners winning right now share one trait: a profitable niche. LTC-at-home beds pay $2 to $5 more per script for the same dispensing work, use your existing wholesaler contract, and sidestep DIR fees entirely. One niche, executed well, beats ten service lines on paper.

Excerpts are from a draft issue. The full issue publishes after launch.

Questions

Straight answers.

Yes. The newsletter is free for readers. Revenue comes from sponsors: clearly labeled ad units from companies that want to reach independent pharmacy owners. Sponsors never touch the editorial copy.
NCPA is the trade association: it advocates, and it sells sponsored email blasts. The Indie Script is vendor-neutral and independent of any association. We cover the fights, including the ones where the news is uncomfortable.
A small editorial operation built on trade press, primary documents, and owner-sourced tactics. Every factual claim is cited to a real source, and the grounding rules are published with the operation.
One issue a week, readable in about five minutes. If it ever takes longer, we failed.
Sponsors buy labeled ad units at published rates, once the list is live. Sponsorship never includes editorial input, pre-publication review, or placement in the copy. Contact us after launch.
